If you would a film review of your surgical case, please follow these steps:
- Download the medical history form and pain diagram. These can be filled out on your computer and emailed to us at espine1@aol.com, or printed and mailed with your films. Here are instructions for the Medical History form.
- Request that your imaging facility provide you with copies of your x-rays, MRIs, CT scans, and any other diagnostic tests performed. Please send the films and the reports. We can accept CDs or films.
- Send your films and medical history via a traceable means such as Federal Express or UPS to:

questions regarding your bill from Dr. Pashman should be directed to

All questions regarding your bill from the hospital should be directed
to Customer Service at 323-866-8600, Patient.Billing@cshs.org, or handled
through the On-line
business office.
Hospital Parking: If
you are in the hospital more than three days, your family may purchase
a parking pass at a reduced rate for parking garages 1, 4, and Lot

at the parking office located in Building 7, room 100. Office hours
are M-F 7:30 am to 4:00 p.m. Phone number: 310-423-5535.
Hospital Visiting Hours: The hospital visiting hours are from 10:00 a.m. to 9:00 p.m.
- Please limit your stay to 15-20 minutes.
- Maximum number of visitors in the room is two at a time.
- Children under the age of 12 are not permitted in the patient's room, nor may they wait unattended in the waiting areas.
- A caregiver may interrupt your visit during some patient care routines.
- If you are ill, please do not visit.
