Office Hours:
Office hours are Monday-Friday, 8:30am-4:30pm. If you have a life threatening emergency, please call 911. Messages may be left with the Answering Service 24 hours per day.
Our offices and the hospital are equipped with free wireless service for patients and visitors.
Prescription Refills:
Please have your pharmacy fax your prescription refill request to us at (310) 423-9963. Prescriptions must be filled during normal business hours. Please allow 24-48 hours for prescriptions to be authorized by the physician and refilled.
New Patient Forms: You may fill out your Medical History prior to your appointment with Dr. Pashman. Please download the medical history form and the pain diagram, fill them in on your computer, and then email them to espine1@aol.com or fax them to 310-423-9963. Please note that the medical history form cannot be saved on your computer. Once you fill it out, either hit the submit or print button. Medical History Trouble Shooting.
Second Opinions: If a physician has told you that you require surgery and you feel as though you would like a second opinion, this service is for you. Please note our Doctors only perform surgery in the Los Angeles area.
If you would a film review of your surgical case, please follow these steps:
- Download the medical history form and pain diagram. These can be filled out on your computer and emailed to us at espine1@aol.com, or printed and mailed with your films. Here are instructions for the Medical History form.
- Request that your imaging facility provide you with copies of your x-rays, MRIs, CT scans, and any other diagnostic tests performed. Please send the films and the reports. We can accept CDs or films.
- Send your films and medical history via a traceable means such as Federal Express or UPS to:

The charge for a 2nd opinion is $125, payable by check or credit card.

Billing Questions: All questions regarding your bill from Dr. Pashman should be directed to Medical Billing Advocates @ (714) 635-3428. You may also contact Medical Billing Advocates for insurance verification or cash prices.All questions regarding your bill from the hospital should be directed to Customer Service at 323-866-8600, Patient.Billing@cshs.org, or handled through the On-line business office.
Hospital Parking: If you are in the hospital more than three days, your family may purchase a parking pass at a reduced rate for parking garages 1, 4, and Lot 2. Click here for a parking map. Parking passes are available at the parking office located in Building 7, room 100. Office hours are M-F 7:30 am to 4:00 p.m. Phone number: 310-423-5535.
Hospital Visiting Hours: The hospital visiting hours are from 10:00 a.m. to 9:00 p.m.
- Please limit your stay to 15-20 minutes.
- Maximum number of visitors in the room is two at a time.
- Children under the age of 12 are not permitted in the patient's room, nor may they wait unattended in the waiting areas.
- A caregiver may interrupt your visit during some patient care routines.
- If you are ill, please do not visit.

Hotel Directory: Here is a list of local hotels for patient's traveling to Los Angeles for treatment. The hospital has a discounted rate with these hotels. Please be sure to mention that you are a patient when you make your reservations.
